BAŞVURU İÇİN GEREKLİ BELGELER

1- Adayın 2008 yılına ait ÖSS sonuç belgesi fotokopisi (ÖSS sonuç belgesinin aslı kayıt sırasında ibraz edilecektir.)
2- Beden Eğitimi ve Spor Yüksekokulu Özel Yetenek Sınavına katılacak adaylar için sağlık raporu (Resmi sağlık kuruluşlarından veya özel hastanelerden “Beden Eğitimi ve Spor Yüksekokulları Özel Yetenek Sınavlarına Katılmasında Sağlık Açısından Sakınca Yoktur” ifadesini içeren rapor.)
3- 4.5 x 6 (cm) ebadında son altı ay içinde, önden, başı ve boynu açık, (erkek adaylar için sakalsız), adayı kolaylıkla tanıtabilecek şekilde çekilmiş 3 adet fotoğraf.
4- Milli sporcular için, Milli Sporcu Belgesi (GSGM Milli Sporcu Belgesi Verilmesi Hakkındaki Yönetmeliğe uygun biçimde düzenlenmiş olması zorunludur. KKTC uyruklu adayların Milli Sporcu belgesi bu sınavlar için geçerli değildir.)
5- T.C.Kimlik numarası yazılı Nüfus Cüzdanının önlü arkalı bir adet fotokopisi, (Nüfus Cüzdanı aslı kayıt sırasında ibraz edilecektir.)
6- Diploma Fotokopisi (Diploma aslı kayıt sırasında ibraz edilecektir.)
1., 5., ve 6. maddelerde asıllarının ibraz edilmesi gerekli olduğu bildirilen (Sınav Sonuç Belgesi, Nüfus Cüzdanı ve Diploma) belgelerden herhangi birinin, fotokopi ve asıllarının birbirine uymaması halinde, bu belge sahibinin sınavı kazanmış olması durumunda dahi kaydı yapılmayacakve hakkında Kanuni işlem yapılacaktır.
